It’s important to remember that Plan Management is not automatically included in your NDIS plan; you need to specifically request it. If Plan Management wasn’t part of your initial plan, you can ask for an early plan review and make sure to mention that you want Plan Management included.

The good news is that you don’t need to justify why you want Plan Management, and there are no eligibility requirements for it. To summarize, simply request Plan Management during your plan review, and it will be added to your NDIS plan. and we can assist you with it.

Our transitional care programme helps participants transition from hospital to home, with a focus on medication management and follow-up care.
